Request letter to the bank disallowing an authorized person not to withdraw money in the bank?

I am searching for the best sample letter that would suit to my problem. I have an employee who knows my account number and my problem is to hold my money in the bank. I don't want her to withdraw any amount from my account. What would I do? What type of letter would I file to the bank so that my employee can't withdraw.
